What should an evidence tracker include?
- Requirement or concern: Record the issue, standard or improvement priority in clear language.
- Owner and commitment: Assign responsibility and define the next action.
- Deadline and status: Record the due date and flag work needing attention.
- Evidence reference: Link to a dated, relevant document or record rather than relying on an unsupported completion claim.
- Verification: Identify who reviewed the evidence and whether the action can be considered closed.
- Impact and follow-up: Record what changed and any further action needed.
Six questions for reviewing evidence quality
- Does evidence exist?
- Is it current, authentic and relevant?
- Does it demonstrate implementation rather than intention alone?
- Does it help explain the result or impact?
- Could staff or learners corroborate the practice, where appropriate?
- Could a reviewer observe the practice or verify the record?
These are practical internal review prompts, not a substitute for the official criteria of any accrediting organization.
Common mistakes to avoid
A policy is not proof that a process operates consistently. A completed task is not automatically evidence of impact. Avoid collecting excessive files without clear owners, dates, context, or verification. Protect personal information and follow your school's safeguarding, privacy, record-retention and reporting requirements.
